Hospitalist (Fully Remote) - Transition Care Management - Nurse Practitioner (Sayre, PA) New

Guthrie is looking for an experienced NP to join its expanding team.

Highlights of the Opportunity

• Traditional Monday - Friday schedule with 8-hour days
• Remote position with occasional on-site coverage as needed 
• Participate in the management of post-discharge care to patients treated by the Hospitalist team
• Collaborate with Hospitalists, nursing team, and central scheduling for continuity of patient care 
• Opportunity for moonlighting if desired

Work-Life Balance

• Fully remote position with occasional on-site care required
• Traditional Monday through Friday schedule 
• No call, no weekends, no holidays
• Paid Time Off annually in addition to holidays and sick time

Candidate Requirements

• 1+ years of experience practicing in an inpatient setting
• Must be a resident of NYS or PA
• Unwavering commitment to our mission of improving health through clinical excellence and compassion; every patient, every time
• Graduate of accredited Nurse Practitioner program 
• Ability to obtain/maintain Pennsylvania and New York state license
