Position Title: IHA Per Diem Nurse Practitioner / Physician Assistant

Start Date: 10/1/2025

Duration: 3 month contract (could be extended)

Schedule: Flexible/Create your own schedule

Travel: Required within a 50-mile radius

Visit Volume: 25–30 visits/week

Compensation: Market rate per completed assessment; mileage reimbursed after first 30 miles/day at IRS rate

Preferred Experience

• Primary Care Provider (PCP) experience
• In-home health experience (required within the last 5 years)
• 1+ year of in-home assessment experience (highly preferred)
  
  

Credentialing Requirements

• Must be credentialed with:
• Excellus
• NYS Medicare
• Medicaid
  

Position Summary

Conduct in-person, no-cost annual preventive health risk assessments for Medicare, Medicaid, and ACA members in their homes. Assessments are 60–75 minutes and scheduled flexibly during the day or evening.

Essential Responsibilities

• Complete self-paced and instructor-led training within the required timeframe
• Provide a minimum of 30 appointment slots per month
• Submit chart notes within 24 hours; respond to queries within 48 hours
• Maintain high-quality documentation and member satisfaction
• Attend periodic training and team meetings
• Comfortable entering members’ homes and performing physical assessments
• Able to lift 20 lbs and bend/kneel as needed

• Proficiency with EMRs and strong computer skills
• Understanding of CMS and ACA risk adjustment models
• Strong verbal and written communication
• HIPAA compliance and confidentiality
• Critical thinking and adaptability

Education & Licensure

• Master’s or Doctorate from an accredited NP program (FNP, AGNP, ANP) or PA program
• Active RN and NP or PA license in the state of practice
• Board certification (ANCC, AANP, or NCCPA)
• Current Medicaid ID and Medicare PTAN
• BLS certification
• Valid driver’s license and proof of auto insurance
• Autonomous License
