Job Details

We are seeking four (4) full-time, field-based Pediatric Hospice & Palliative Care Nurse Practitioners to join our expanding care team. This role focuses on delivering high-quality, compassionate care to pediatric patients enrolled in hospice and palliative care programs, while supporting families and collaborating closely with physicians and interdisciplinary team members.

The Nurse Practitioner will provide care in community, long-term care, and hospital environments, utilizing both in-person and telehealth visits as appropriate.

Available Territories:

Brooklyn – (car required)

Queens – (car required)

Bronx – (car required)

Manhattan – (car not required)

Responsibilities:

  • Provide NP-level hospice and palliative care services to pediatric patients
  • Conduct face-to-face evaluations for hospice eligibility
  • Manage urgent and complex medical needs for hospice patients
  • Deliver expert symptom management and goals-of-care discussions
  • Collaborate closely with physicians, managers, and interdisciplinary teams
  • Provide in-person and telehealth visits in alignment with best practices
  • Coordinate care plans, documentation, and treatment implementation
  • Support and educate hospice staff on pediatric hospice care
  • Assist in developing pediatric hospice education teaching curriculum for cross-coverage teams
  • Apply evidence-based practices, policies, and procedures
  • Participate in quality improvement initiatives as needed
Qualifications:

  • Family Nurse Practitioner (FNP) with at least 1 year of experience as a provider
  • Experience treating pediatric patients as an FNP or as a staff RN
  • Pediatric Nurse Practitioner (PNP) experience with terminally ill children strongly preferred
  • Strong clinical judgment, communication, and collaboration skills
  • Ability to work independently in a field-based role
  • Valid driver’s license and reliable vehicle required for Brooklyn, Queens, and Bronx roles
